    For the Create Collection form in the Admin>Manage section, open plog-admin-functions.php and look for line 1472:

    <form action="'.$_SERVER['PHP_SELF'].'" method="post">
    Who is your webhost, and what did your webhost tell you about this issue?

    Can you click the Show Server Info link in the upper right of the screen in the Admin section and post your server data here so we can try to figure out why you are getting this error.
